The FSX aircraft are not correctly mip-mapped and because of that traffic creates a huge hit on resources. Once the system is smooth you can try raising AitlineDensity a but however using a replacement AI fleet will allow the traffic sliders be raised to a more reasonable value such as 80-85%

Try the above and see if it makes a difference. What you are looking for is consistant frames and as you decend into a populated busy area, maintaing smooth operation. Forget about the frame rate (leave it off) and focus on how it looks and feels. The frame counter robs the sim of 3-4 frames of performance when it is displayed and is off by as much as -5 frames. @ 18, its actually about 22-24.
